Health Insurance Card(Hokensho)
If you are to move in, move out, or move within the city, you need to report the change of address to the Civic Affairs Division within 14 days from the date of your move.


Notes:
1. Your National Health Insurance Certificate must be returned to the Health Insurance and Annuity Division either in person or by mail when you move out of Kamagaya City.

2. If you move your house out of the city and you have to change your child’s school, you’ll need to file a report of the change with the city hall with the Hokensho health insurance card and the student ID card of your child.

3. When you enroll in or withdraw from your national health insurance, or there is a change in the insurance, you must file a report to the city within 14 days from the date of change.

Admission to or change of schools
Changing schools due to move

1. Moving to Kamagaya City
You will need to file a report of your move to Civic Affairs Division and get a “School Transfer Notice – Entrance Form”, which will be required at your child’s school for admission.

2. Moving out of Kamagaya City
You will need to report your move to Civic Affairs Division and get a “School Transfer Notice – Withdrawal Form”, with which your child will get a “Student Certificate” and “Textbook Grantee Certificate” at the child’s previous school.

3. Moving within Kamagaya City
In case of the intra-city school transfer, you will need to report your move to Civic Affairs Division and get a “School Transfer Notice – Withdrawal Form” and a “School Transfer Notice – Entrance Form”, which will be required in leaving the previous school and entering a new school.

Electricity
Electricity supplied to ordinary Japanese homes is 100 volts and 50 Hz. When moving from an area with different voltage or frequency, be aware that some of your electronic appliances may not function.
All houses and apartments in Japan are equipped with a circuit breaker in order to prevent accidents related to electricity.

When your appliances do not work
When you use more electricity than the contracted amperage, the circuit breaker automatically shuts off the power supply. Turn off the switch of the appliance which may have caused the cutoff, and turn on the breaker switch to restore the power supply.

The circuit breaker also breaks the circuit in case of short circuit resulting from mechanical trouble or damaged cords. Unplug the cord of the appliance and turn on the breaker switch to restore the power supply.

Gas
The only city gas (Type 13A) supplied in Kamagaya City is through Keiyo Gas Company. Contact the gas company to start using gas or terminate gas supply ahead of your move.

Moving in:
Ask the gas company to come and open the gas valve and check your gas appliances for compatibility and safety before using them.

Moving out:
Report your move to the gas company to terminate the gas supply ahead of your move.

Post Office
If you report the change of address to your nearest post office, all the mail delivered to your previous address will be forwarded to your new address for free for one year from the date of your report. To lodge a change in address please bring something to verify your identity such as driver’s license, passport, or health insurance card to the post office.

Bank
You are required to report the change of address to your bank.

Driver’s License
Any change of information on your driver's license must be reported to the local police station or driving test center. To register your new address, bring your foreign registration card or other documents with you that verify your name and new address.

Newspaper
To continue your newspaper subscription at the new address, inform your paper shop of the change of address.
